The star, 53, pulled down her trousers to expose her derriere - under a G-string and lacy tights - to concert-goers in Rome, the Daily Mail reported.
The Queen of Pop first gave an eyeful, peeling down one of her bra straps, while singing Human Nature during a show in Turkey.
During the same song in the Italian capital, she showed off her toned bottom.
The Material Girl, who is due to perform in London next month, is currently on her MDNA world tour, which kicked off in Israel this month.
