Four months ago, a team of inspectors arrived unannounced at a small hospital for extremely vulnerable patients in Londonderry. What they discovered shocked them.

Within days, the Western Health Trust, which is responsible for the facility, privately feared that it might be ordered to shut Lakeview Hospital, which was only built in 2005. The case quickly went to the very top of the trust and to the top of the health regulator, the RQIA. Yet to this day, relatives of those cared for in Lakeview have not been told of what inspectors found.
